Sacubitrilat is the active metabolite of sacubitril and serves as a neprilysin inhibitor in the treatment of chronic heart failure. It is a key component in the fixed-dose combination drug sacubitril/valsartan, marketed under the brand name Entresto, where sacubitril is rapidly converted to sacubitrilat in the body. By inhibiting neprilysin, sacubitrilat increases levels of natriuretic peptides, promoting vasodilation, natriuresis, and diuresis, which reduce blood pressure and fluid overload. This mechanism improves cardiac function, manages symptoms, and lowers the risk of hospitalization in patients with heart failure with reduced ejection fraction.
